\Tangshan (played by Donnie Yen) and Liu Guobang return from studying medicine in the UK and open a clinic in Zhabei District, Shanghai to help the villagers. One day, the local Axe Gang comes to cause trouble, intending to occupy the village land to build a casino. Tangshan and Ah Bang cannot stand it, so they intervene, which leads to a conflict with the gang leader Yu Laoqi (played by Yu Rongguang). Tangshan learns that the jurisdiction of this land belongs to his teacher Dr. Raymond, so he asks Raymond for help, causing the Axe Gang's plan to fail. Yu Laoqi is superstitious about the fox spirit and does not allow his sister Yu Xiaoqian (played by Athena Chu) to receive vocal cord treatment at the hospital, causing her to be unable to speak for fifteen years. Xiaoqian and her friend Ye Ling (played by Jiang Hong) go to the hospital for a check-up and happen to meet Tangshan after his surgery. Tang invites Xiaoqian to the clinic for treatment when she is free. Xiaoqian undergoes surgery at the hospital, and Yu Laoqi rushes there with people to stop it when he hears the news. Yu gives Tangshan only seven days to cure Xiaoqian, and after seven days, Xiaoqian can finally speak. A case of a missing child occurs in the village, and after finding the body, it is discovered that all the internal organs have been removed. Tangshan and Ah Bang swear to investigate to the end. One night, Ah Bang accidentally discovers that the kidnappers are members of the Axe Gang and follows them, but is caught and beaten to death. Tangshan finds a jade pendant in Ah Bang's hand and asks Xiaoqian, learning that it belongs to Yu Laoqi. It turns out that Dr. Raymond is colluding with the Axe Gang; he takes the children's organs for experiments and frames Tangshan. Tangshan defeats the pursuing police and escapes. Xiaoqian learns the news and leaves home to search for Tangshan.Yu led his men to chase Tang Shan, and in the chaos, his subordinate A Long killed Xiao Qian. Tang Shan gathered evidence of Dr. Raymond's crimes and prepared to report it. Yu wanted to confront Tang Shan alone to settle their feud, but Raymond ordered him to steal back the evidence and spoke disrespectfully, and in a fit of rage, Yu killed him. Yu blocked Tang Shan to duel, but still died to his own flying axe.
