It's very simple. When he was rescuing the girl, he got tangled in aquatic plants. However, when revisiting the place, the old man said while fishing that there were no aquatic plants here. So, the man got tangled in the girl's hair when trying to rescue her. He did not save the girl but instead caused her death.
That aquatic plant is actually his girlfriend's hair. He indirectly killed his girlfriend, which counts as involuntary manslaughter, and in the end, he was mentally broken and committed suicide to accompany his girlfriend.
